Uso y abuso de los psicofármacos

Seguramente todos conocemos a alguien cercano a nosotros (amigo, compañero de trabajo, familiar, vecino, etc,) que está tomando algún tipo de ansiolítico o antidepresivo, o «pastilla para dormir».

El autodiagnóstico en cuanto hay algún tipo de interferencia emocional negativa y el acudir al médico de atención primaria (en el mejor de los casos) para solicitar y conseguir la medicación está por desgracia a la orden del día.

Se hace eso antes que probar alternativas que están más a nuestro alcance, como incorporar hábitos estables y más saludables que pueden ejercer un beneficio similar o incluso mejor que el que buscamos, como pueden ser mejorar nuestra alimentación, regular el sueño, hacer ejercicio regularmente, practicar respiraciones y relajación, realizar actividades que nos resulten agradables en nuestro tiempo de ocio, etc.

 O en los casos donde hay síntomas más incómodos o importantes, plantearse acudir al psicólogo, contarle lo que nos pasa, y recibir la orientación de un especialista que, sin duda alguna, nos va a ayudar a resolverlo de manera estable y duradera, sin tener que depender fisiológicamente de nada, enseñándonos a poder mejorarlo nosotros, de manera natural, sin química artificial, incluso aprendiendo a cómo potenciar la neuroquímica que de manera normal y cotidiana funciona en todos nosotros haciéndonos sentir mejor o peor.

El uso y abuso de los psicofármacos más comunes se está convirtiendo en algo normalizado, incluso ya en población adolescente e incluso infantil, con el riesgo de efectos secundarios colaterales que ello conlleva, y sin que sirvan en la mayoría de los casos para conseguir una mejoría emocional y conductual en la persona.

Es cierto que en algunos casos es necesario un aporte para conseguir reducir sintomatología aguda que está bloqueando nuestra capacidad de acción y nuestro bienestar, pero no lo es menos que, otras veces, cuando este aporte es innecesario, lo que se puede producir es un efecto negativo. A lo mejor conseguimos distanciarnos emocionalmente, sufrir menos y que no nos importe lo negativo, pero puede que a cambio consigamos una dificultad de concentración  o atención en nuestras actividades diarias, o menor capacidad de disfrutar de las cosas, o alteraciones fisiológicas colaterales (comida, sueño…) o disminución de libido o placer sexual, etc.

En efecto, hace muchos años ya que infinidad de estudios han demostrado la eficacia de fármacos en la mejoría de diversos síntomas psiquiátricos, como pueden ser la ansiedad, depresión, conductas obsesivas e impulsivas, etc. Por no hablar de los de mayor gravedad, como son alucinaciones, delirios, y todos los que forman parte del ámbito de las psicosis. Ahora bien, también sabemos hoy en día que para que estos fármacos puedan producir un efecto beneficioso, deben llegar en cantidades adecuadas al cerebro, y que esto depende, además de cumplir con las pautas del tratamiento indicado por un especialista (en este caso un psiquiatra), de su absorción, distribución, metabolismo y eliminación. Cómo actúe finalmente dependerá de su mecanismo de acción, de su farmacocinética y de su farmacodinámica. Y por supuesto dependerá también de las características fisiológicas individuales de la persona a la que se le administra, y también del momento y circunstancias emocionales en los que se hace.

En un marco psicoterapeútico, que es donde deberían aplicarse, sólo se decide su aplicación después de haber realizado una evaluación exhaustiva de la naturaleza, cualidad e intensidad de la sintomatología presente, y de la existencia o no, según la historia vital de la persona, y de la circunstancia actual, de otros recursos a disposición de la persona o que se puedan desarrollar con ayuda del psicoterapeuta y que van a resultar cuando menos igual o más eficaces en muchos casos que la medicación ( vínculo terapeútico, técnicas cognitivas, habilidades de afrontamiento, sistema de apoyos, habilidades asertivas y sociales, resolución de problemas y autocontrol y regulación emocional, autoconcepto y autoestima , habilidades de comunicación y relacionales, etc.).

En conclusión, estamos a favor del uso de psicofármacos siempre que esté indicado dentro de un contexto terapeútico interdisciplinar, siempre que sea necesario dentro de un tiempo más o menos limitado y ajustándose a las características individuales presentes.

Y en contra del uso indiscriminado y generalizado que se hace de ellos, en lugar de buscar opciones más sanas y adecuadas.
